Non-Medi-Cal patients are screened financially for Medi-Cal, and MISP eligibility. Ability to pay program (sliding fee) is available for low-income patients who provide proof of income.

Pediatric Immunizations:
Assessment of Immunization level. Immunizations given for Polio, Measles, Rubella, Tetanus, Diphtheria, Pertussis, Mumps, Hepatitis B, HIB, Varicella, Hepatitis A, Human papilloma virus (HPV), Pneumovax.
Adult Immunizations:
Hepatitis B, Flu, and Pneumovax

Pediatric Immunization: For all children under the age of 18, parent or guardian consent required. Fees charged.
Walk-in services at Family Care Centers. Low fee-maximum set per family. No one is denied service due to inability to pay.

Anonymous and Confidential HIV Testing:
Counseling and Referral.

HIV/AIDS Primary Care:
Evaluation, treatment and follow-up, provided as part of expanded sexually transmitted disease services.

Services will be offered to those individuals with a confirmed HIV+ test, or who are exhibiting symptoms associated with an HIV infection, or people concerned about the possibility of having symptoms.  All services are provided on a confidential basis.  Fees are charged.  Medi-Cal and private insurance are accepted.  For other low income patient’s fees are based on an ability-to-pay determination with a sliding fee schedule, according to income.

Tuberculosis Testing:
Skin testing using the interdermal PPD. A return visit for reading is required. Chest X-Ray evaluation for those with a positive reaction.

For a suspected case or contact to an active case or for children:  fees are charged.  Medi-Cal and private insurance are accepted.  For non-Medi-Cal low-income patients, fees are based on an ability-to-pay determination with a sliding fee schedule.  No one is denied services due to an inability to pay.    For work related testing:  a set fee is charged.  There is no sliding scale.  Payment is due at the time of service unless contract, voucher, or other arrangements for billing have been made.

Tuberculosis Clinic:
Clinic evaluations of TB patients to include medication, X-ray, skin test. Laboratory testing includes sputum and blood work.

Treatment of latent TB infection is also available.

Appointments for physician evaluation coordinated by Disease Control.

Sexually Transmitted Diseases (STD):
Examination and laboratory testing for syphilis (any stage), Hepatitis B Screenings, gonorrhea (any site), chlamydia, trichomiasis, ano-genital warts, education, treatment and investigation of contacts.

Cervical Dysplasia:
Colposcopy, biopsy and cryosurgery services. Referral for surgery if indicated.

Family Planning:
Basic contraceptive services for males and females include physical exams, laboratory testing (including pap smear), STD screening, information, education, counseling, referral, pregnancy testing, birth control methods and counseling.

Family P.A.C.T. state only provider. Medi-Cal and private insurance are accepted. Non-Medi-Cal clients are financially screened for eligibility for State Family P.A.C.T./Federal family planning funding, whereby client fees are based on a sliding fee schedule according to income and family size. No parental consent required, age 12 or older.

Children from birth to 21 years of age who have Medi-Cal, Healthy Families and Healthy Kids. Also, children may qualify for CHDP funding according to family size and income. Any child attending a State licensed Preschool/HeadStart Program is eligible for an examination.

Cancer Screening:
Physical exam including breast and pelvic exam. Education: Self-breast exam.
Lab: Pap smear, hemoglobin, chem., panel and other studies as indicated.
Referral: Mammography and other as indicated.

All women eligible at age 18 or older. Children under 18 need parental consent. Medi-Cal and private insurance are accepted. Non-Medi-Cal patients are screened financially for Medi-Cal or MISP eligibility, BCEDP. Ability to Pay (sliding scale) is available for low-income patients providing proof of income, BCEDP.

Pediatric Immunizations
Assessment of Immunization levels.  Immunizations given for Polio, Measles, Rubella, Tetanus, Diphtheria, Pertussis, Mumps, HIB, Hepatitis B, Hepatitis A,  Human papilloma virus (HPV), Pneumococcal disease, Flu for healthy children 6 months – 23 months and children at high risk, and Varicella (Chickenpox).

No one Is denied service due to inability to pay. Free immunizations are available at Outreach Clinics for children without a medical provider.

Encourage teen parents to stay in or return to high school, or an equivalent program, and earn a diploma or its equivalent.
Long-term, comprehensive case management for pregnant & parenting teens who are CalWORKs recipients. Provides counseling & referral to needed services, advocacy & follow-up. May also receive financial incentives for satisfactory school progress and high school graduation or equivalent and financial disincentives for school failure or non-participation. Supportive services (child care, transportation, and ancillary services) are available if needed for school attendance.

Administrative and Case Management Services
California Children Services (CCS) is a statewide program of specialized medical care and rehabilitation for children with special health care needs. The program provides diagnostic and treatment services including: Hospital admissions, outpatient visits, surgeries, X-Rays and laboratory testing, medicines, durable medical equipment, and other rehabilitation services; coordination of medical care, referrals to other agencies.

Medical Therapy Program
Therapy services are provided to physically handicapped children at medical therapy units located on public school campuses.

In-home services provided by Public Health Nurses include assessment, guidance, counseling, teaching, intervention, referral and family case management in the categories listed below:

Child health, adult/senior health, perinatal, postpartum, family planning, parenting education, (in-home and also group classes in various cities throughout the County), high risk pregnancy, high risk infant, grief/loss, child abuse prevention, adult/elder abuse, communicable disease, tuberculosis, infant death, drug exposed infant, home birth/newborn screening follow-up, homeless, domestic violence, teen parenting, and wraparound services.

Fees are not charged for the services.

Public Health Nurses work in collaboration with the Department of Mental Health, Social Services, Probation and Education to ensure that the health related needs of families are identified and addressed through available community resources. In this partnership the Public Health Nurse provides support individualized to each family’s unique strengths and needs promoting health, safety and permanence in the home, school and community.

Public Health Nurses provide individualized care to meet the needs of first time moms and their babies.
This includes monitoring pregnancy for complications, information on what to expect at the time of delivery and how to care for herself and child at home. The mother and child are visited in their home regularly until the child reaches 2 years of age.

5 A DAY Power Play Campaign
Promotes the increase of consumption of fruits and vegetables in children. To decrease incidences of chronic diseases. Provides educational activities to increase children's consumption of fruits and vegetables are organized through community channels including schools, community youth organizations, farmers markets, and retail. Free educational materials available.

Latino 5 a Day Campaign
To increase consumption of fruits and vegetables among Latino adults and their families. The program uses a combination of culturally and linguistically appropriate mass media and community-based strategies to communicate the 5 a Day and physical activity message.

The 5 a Day Retail Program
To conduct comprehensive promotional and merchandising (point of sale materials and recipe card) activities with retail food outlets to increase purchase of fruits and vegetable among low income Californians.

African American 5 a Day Campaign
Promotional activities for African Americans in Riverside County.

Activities for children ages 9-11 in elementary schools with at least 40% free and reduced meals. Materials and activities are free of charge.

Latino adults and families who are food stamp eligible or recipients.

Food stamp population at 5 a Day partner retail stores and including Albertsons, Ralphs and Vons in our community.

African Americans living in Riverside County.

Services are provided in schools throughout the County.

Intervention at large Latino festivals, farmers/ flea markets and neighborhood stores and education to direct health service provider organizations and community groups.

• Breastfeeding Helpline: Breastfeeding support and assistance available 24-hours per day, 7 days per week, 365 days per year.  Helpline staffed by certified Lactation Educators and Lactation Consultants - 1-888-451-2499.
• Prenatal Education:  Pregnant women are encouraged to call the Breastfeeding Helpline to enroll in the Loving Support Program and request a free packet of breastfeeding information.
• African-American Peer Counselor Project – “Sistah Connection”: Trained Breastfeeding Peer Counselors facilitate monthly breastfeeding and parenting support groups for pregnancy and exclusively breastfeeding women at the following locations:  Riverside (2), Moreno Valley (2), Hemet, Rubidoux, and Perris.
• Individual Care:  Women are referred to hospital-based lactation help clinics when in-person breastfeeding support and help is necessary.
• Inspirational Cards:  Mothers are sent inspirational cards to congratulate, encourage and honor them during their baby’s first year (3 weeks, 3, 6, 9 and 12 months).
• Newsletter:  A quarterly newsletter providing helpful hints and up-to-date information and resources to support the mother and her family to continue breastfeeding.
